Here is a project plan template from the Management Center, a group that works with nonprofits on strategic planning and professional development. They have some other helpful tools on their website in a variety of project management areas. This might be helpful for team leaders: http://www.managementcenter.org/tools/

Here is a template for a simple goals and activities chart from University of Washington. Elizabeth has used similar tools in the past, and it's easy to plug into Excel or whatever program project leaders prefer. 

The final template (see attachment) is a similar template Elizabeth found from Vanderbilt University.
